Programs
Through funding provided by the Congressional Black Caucus(CBC) South Side Help Center, in partnership with the Clinic in Altgeld has received a grant from the Centers for Disease Conrol to reduce the incidence of HIV, STD's, TB and substance abuse among African Americans in the Greater Roseland community. Because of the great need for collective services Ujima Coalition (Ujima is a Swahili word that means to build and maintain our community together, collective work and responsibility) was formed. The Ujima Coalition is a network of providers serving the Roseland and surrounding communities.
The mission of Ujima is to design a plan is to guide and strengthen linkages among the Ujima member agencies that provide HIV, STD's, TB and substance abuse prevention and treatment, care services and other health and social service programs; and to provide coordination, tracking and referral of clients in the African American community.
Fourteen health and social service providers offering a wide range of services comopose the Ujima Coalition. Partners in the coalition include Altgeld Health Center, Christian Community Health Center, Circle Family Services, Community Supportive Living Systems, Human Resource Development Institute, Hymn of Fellowship, LePenseur Youth & Family Services, Roseland Woodlawn Organization / Coach House, Unity Parenting & Counseling, and Universal Family Connection with South Side Help Center serving as the lead agency.
The communities targeted by the project are: Roseland, Pullman, West Pullman and Riverdale.

South Side Help Center serves as the lead agency and administrative core of MOCHA (Men of Color Collaborative for Health Care Alternatives). MOCHA is a Chicago initiative comprised of eight community based organizations (CBOs), funded by the Office of Minority Health (OMH). Members of MOCHA include CALOR, a division of Anixter Center, Howard Brown Health Center, Horizons Community Center, Jelani Unified Men’s Project, Minority Outreach Intervention Project, TaskForce AIDS Prevention, Test Positive Aware Network and Vida/SIDA. The mission of this collaborative is the improvement of health care delivery to African American and Latino men who have sex with other men (and their partners) and who are at risk for HIV infection.
The goal of this initiative is the successful implementation of an Organizational Case Management (OCM) system that includes developing and implementing a model mentorship approach which pairs local, regional, and national health organizations with minority community-based organizations (M/CBOs) and minority-serving community-based organizations (MS/CBOs) to develop fiscal and programmatic viability, so that they may be better poised to successfully compete for additional federal and private funding support. This program assists youth-parolees with various human service programs to enable their successful reintegration into the community. This program provides improved social and educational services for juveniles and other male youth. Through mentorships, counseling, education services and skills development, this program helps to increase youth parolees' rates of success to transition back into society. This program is designed to ensure public safety for Illinois residents, reduce recidivism, and redirect the course of youth to rebuild their lives.
The South Side Coalition is comprised of South Side Help Center and Task Force AIDS Prevention and provides a social support network for African American men who engage in high risk behaviors and are considered difficult to access. The coalition encourages healthy decision-making and presents positive alternatives by providing HIV/AIDS prevention education/outreach, street and fixed site contacts, social events and condom and literature distribution.
This project is designed to encourage healthy decision making and present realistic alternatives for adult African American males who engage in high-risk behaviors by providing specially targeted HIV/AIDS prevention and education information. This initiative is designed to reach men who may be difficult to access and whose circumstances may lead to involvement in high-risk behaviors. The program provides outreach services in various venues such as: churches, nightclubs, bars, shelters, and other locations where adult men are found.
The focus of Kujipenda (self love) is to raise the awareness of African American women ages 10-24 regarding behaviors which place them at risk for HIV/AIDS. Through sexual health discussion groups and prevention case management, the program seeks to empower women by providing them with a confidential setting of peers and staff to discuss their concerns. The program provides accurate HIV and STD prevention information, knowledge, and skills building to increase participants' confidence in making positive choices regarding their sexual health.
